The focus of the appraisal team was formed of two major areas: Health Account and earmarked support. The work comprised:
  1. Assessment of critical aspects of the POW II with a view to continued support through the Health Account:

    • Monitoring of the programme

The Ministry of Health has launched a Health Sector Reform Project (HSRP). This programme aims at organising the health services towards all families and being financed by health insurance (compulsory and complementary health insurance). A pilot model of health services is being tested in 5 Governorates, including both purchasers and provider organisations. Reform of the present health insurance scheme and the development of sustainable financing strategies are key elements of the reform.

Description of Actual Services Provided:
Regular monitoring of the reform process through bi-annual evaluations. Advise to the Minister of Health on the reform strategies and implementation.

The Ministry of Health has launched a Health Sector Reform Project (HSRP)). This programme aims at organising the health services towards families and being financed by health insurance. A pilot model of family health services has been created in 3 Governorates. This model has been operated by the Ministry of Health, the Health Insurance Organisation (HIO) and the private sector. The Ministry of Health wishes to develop a health insurance system and to reach national coverage.

The European Commission provides support to this reform through budgetary help for the Family Health Fund, annual activity programmes and technical assistance programmes. Moreover.

Description of Actual Services Provided:
An external team monitors the process of the reform and gives advise to the Ministry of Health and the EC to improve the impact of the reform strategies. HERA provided the team leader to the monitoring team. Bi-annual missions were carried out between 2001-2006.
  • Monitoring, on behalf of the EC, the progress of the health sector reform and health insurance reform in Egypt (over a 5-year period);
  • Advise the Ministry of Health and Population on reform concepts, policy development, processes and change;
  • Advise the EC on Tranche Release for the sector reform.

The objective is to assist the Province of Luxembourg in deciding on an appropriate course of action, the objective of this particular assignment is to make the ‘case for change’. Within the scope of meeting the economic and financial objectives, the ‘case’ shall build on current international trends and experience in hospital service rationalisation and take the Province de Luxembourg’s particular circumstances into consideration. The ‘case’ may include one or two options to the Province’s current hospital configuration - broadly elaborated and based on relevant, applicable practice. To further assist in decision-making, the assignment will include a ‘road map’ setting out the steps, the work, the timescale, and the likely cost of identifying and fully appraising a preferred hospital configuration for the Province.

  • a basic survey of the health institutions in PdL – to assemble the essential knowledge of existing institutions’ capacity, functionality, services offered, staffing, catchment area, design, state of repairs, management, and financial situation etc;
  • a broad assessment of the demand for hospital services in the Province, based on epidemiological data or deriving from a similarly accepted methodology;
  • an estimate of required hospital capacity in the Province. The estimate will take due note of current international trends in hospital services provision and - to the extent possible – take into consideration PdL inhabitants’ access to services outside the Province and the need to continue to procure certain specialised services elsewhere;
  • an analysis of the ‘problem’ and a reasoned suggestion on a response. The pros and cons of the option(s) as concerns investment cost, recurrent cost, accessibility, quality of services, staff requirements, etc. should be described in approximate terms comparative to the current situation;
  • a ‘roadmap’ of the of how to move towards identifying and selecting a preferred option. This will include a detailed description of the tasks included, their time scale, and their approximate cost (=terms of reference), and advise on other practical steps in the process.

Assignment: Evaluation of a Public Private Partnership arrangement for a new national referral hospital

The Government of Lesotho engaged the International Finance Cooperation (IFC) to act on its behalf on all aspects of bidding that includes the construction of the hospital according to specifications, the tertiary clinical service package, human resource issues and all contractual aspects. IFC has been working on this assignment since early 2006 and has now come up with draft bidding documents that have shown the way forward in terms of options for what services can be developed at the New Referral Hospital against the resource envelope that the Government of Lesotho can afford. It is in this context that a short term independent Technical Advisor was sought to advise the MOHSW on the aspects of the PPP that have been put forward by IFC that would optimally suit the country in providing the best tertiary referral services at the new hospital within the affordability frame of government.

Description of actual services provided:
  1. Study the documents produced by IFC, specifically the draft bidding documents, and all other relevant documents on QEII Hospital.
  2. Study the financial proposals put forward by IFC for their soundness in regard to the construction costs and running costs of the new hospital and Unitary Payment costs obligations required by government
  3. Advise on the scope of service that is proposed by the IFC draft bidding documents compared with what the clinicians at QE II are proposing
  4. Advise on the human resource option for provision of clinical services as contained in the IFC bidding documents.
  5. Advise on the option of including the Filter Clinics of Maseru City catchment area in the PPP arrangement.

The Government of Botswana itself financed a study to review the organizational aspects of its Ministry of Health.

Description of Actual Services Provided:
  1. Examined the Ministry's Mandate, Vision, Mission, Ideals and Strategies in the context of Vision 2016 and NDP8, and suggested improvements if necessary.
  2. Identified Ministry strengths and weaknesses, taking into account the current needs of the Health Sector.
  3. In the process of the above analysis, examined the feasibility of decentralisation of appropriate functions to the regions/districts to facilitate service delivery and customer empowerment. In the same purview, assessed the adequacy and distribution of the Ministry's functions and, where feasible and practicable, relocation, realignment and/or privatisation or outsourcing some of them.
  4. Determined reporting relationships and co-ordination within the proposed structure both within Government structures, Local Authorities, Mission Hospitals, non-government health-related organisations, and to propose improvements.
  5. Given the above scenario, examined and streamlined the Ministry's functions and organisational structure.
  6. Explored areas requiring computerisation systems, and made appropriate proposals.
