This mission has two principal objectives:
  • collect data, verify the results and analyse the constraints to establish the efficiency and effectiveness of the use of available resources in the framework of the implementation of the 7th framework-plan;
  • provide an organisational analysis of the existing organogramme of the administrative procedures, the management tools of the participation of the population in order to establish the effectiveness in the health programme of the BELACD of Pala.
Description of Actual Services Provided:
The expert implemented the evaluation, in particular the review of the results, the achievement of activities and the management of the health programme. In a second phase the expert provided recommendations and conclusions regarding the actions to be implemented when elaborating the next triennial plan.

The objective of this mission is twofold: On one hand the evaluation of the support programme of Memisa to the district of Mibilizi and on the other hand the preparation of a 3 year programme including the support to the districts of Mibilizi and Kibogora.

Description of Actual Services Provided:
The team did the evaluation of the support programme of Memisa, including questions regarding the objectives, indicators, budgets, needs of the population, the strategies, the coordination, etc. Recommendations were included in the report and formed the basis of the formulation of the fulltime 3 year programme. The team held a restitution meeting with the partners and the representatives in order to share the conclusions and recommendations.

  • To plan and implement a major capital investment programme covering all areas of MSL operations
Description of Actual Services Provided:
  1. Make a full assessment of MSL’s QA systems and procedures in the context of the national regulatory environment, including detailed analysis of the current strengths and weaknesses not only at MSL but in the national structure as a whole
  2. Make detailed and costed proposals for investments, tools, training and restructuring which is required to bridge the perceived gaps in the QA system at MSL
  3. Include in the above analysis an assessment of the options available to MSL for QA testing of all incoming products, with a focus on drugs and medical supplies. This assessment should include:
    • Analysis of tests required to cover MSL’s entire product catalogue
    • Assessment of all available models which will provide the required tests (e.g. establishment of MSL laboratory, outsource the whole service, etc)
    • Presentation of costed plans for all options and timetables for implementation, and
    • Clear recommendations for the way forward

The general objective of the mission was to evaluate the Programme to Support the Health and Social Development of the Mopti region.

The specific objectives were to:
  • Judge on the relevance of the PADSS in the 5th Region, and on the consistency of the PADSS
    1. with the sectorial health policy (from 2005 on implemented by the PRODESS-2);
    2. with its environment (regional context, other partners).
  • Analyse the institutional organisation, as well as the organisation of the programme, its management, and the role/ performance of the technical assistance SOFRECO.
  • Analyse de state of progress of the planned activities, as well as the extent of the results (including their ‘impact’ on the health system of the 5th Region).

The objective is to provide decision-makers in the government of Syria and European Commission with sufficient information to make an informed judgement about the performance of HSMP (its efficiency and effectiveness), and decisions about any required changes to project scope (such as objectives, duration, financing, management arrangements, etc).

Description of Actual Services Provided:
The monitoring team carries out 2 yearly missions to the project. A separate evaluation team carries out a Mid-Term Review and a follow-up workshop in between the monitoring missions. Both teams advise on the state of implementation and on the further conduct of the project throughout the implementation of the HSMPT (2006-2007).
  • Assess and comment HSMP reports of MoH and the TA team;
  • Assess and comment policy initiatives of MoH or of GoS that are relevant to the health sector (e.g. civil service reform, budgetary changes, changes in management of public finance or public procurement, privatisation policies and redefinitions of the role of the state etc.);
  • Assess and comment economic, societal, political, epidemiological and other changes of framework conditions that are relevant to the health sector;
  • Assess and comment donor coordination by the MoH;

The European Commission (EC), through the 8th European Development Fund (EDF), is financing two Health Sector Support Programmes (HSSP-I and HSSP-II) in the Ministry of Health & Child Welfare (MOHCW) of Zimbabwe. The two programmes are based on the needs expressed in the 1997-2007 Zimbabwean National Health Strategy, which includes an agenda for health reforms in Zimbabwe. HSSP-I started in August 2000 with a budget of €24.5m, with the objective to “promote social justice through improved quality of life amongst the most vulnerable by developing a sustainable and efficient health system”. In February 2002, EU measures against Zimbabwe necessitated a re-programming of the project, which was completed in March 2003. HSSP-I is expected to run until September 2006. HSSP-II started in November 2001 with a budget of €33m, with the objective to “reduce the incidence and alleviate the impact of HIV/AIDS and other priority diseases in Zimbabwe”. HSSP-II is programmed to end 30 November 2004. It was not affected by the EU measures.

For both evaluations the team used conventional components of document review, interviews with key stakeholders, and a workshop for presenting the findings and recommendations as well as gathering feedback. Key stakeholders were identified and a provisional work plan was made. The interviews were semi-structured with questions along the lines of the specific objectives.

The specific objectives for Final Evaluation of HSRDP II were achieved as described below:
  • A comprehensive assessment of the achievements of the programme was performed;
  • The lessons learned were analysed;
  • The relevance and efficiency of the programme management and implementation was evaluated;
  • The effectiveness of planning and management support, monitoring and supervision, communication, coordination, etc were examined;
  • The stakeholders’ participation was evaluated
  • The likelihood for positive outcomes to continue after completion period was assessed.
For the Mid-term Evaluation of the SIHSIP, the specific objective were addressed as follows:
  • The extent to which the SIHSIP is contributing to development in the health sector was reviewed;

The review was empirical and evidence based. It started with some deskwork done at WHO Headquarters followed by in-country visits to the following ACP countries: Haiti, Malawi and Tanzania. These countries are representative of the different geographical areas included within the programme and will permit to get field evidence for the overall programme review.
After the field visits a first draft of the report was presented to WHO, EC and ACP secretariat who submitted their comments to this report.
Version 2 of the report was produced including recommendations proposed by the evaluators. This was discussed during the validation workshop in Nairobi.
A final version of the report, including minutes and final comments made at the workshop will be presented .

The Ministry of Health has launched a Health Sector Reform Project (HSRP)). This programme aims at organising the health services towards families and being financed by health insurance. A pilot model of family health services has been created in 3 Governorates. This model has been operated by the Ministry of Health, the Health Insurance Organisation (HIO) and the private sector. The Ministry of Health wishes to develop a health insurance system and to reach national coverage.

The European Commission provides support to this reform through budgetary help for the Family Health Fund, annual activity programmes and technical assistance programmes. Moreover.

An external team monitors the process of the reform and gives advise to the Ministry of Health and the EC to improve the impact of the reform strategies. HERA provided the team leader to the monitoring team. Bi-annual missions were carried out between 2001-2006.

In July 1999 the House of Representatives of the Netherlands approved the sector-wide approach (SWAp) as method to increase aid effectiveness. In December 2003 the Minister for Development Cooperation announced an overall evaluation of the SWAp. The independent Policy and Operations Evaluation Department (IOB) of the Netherlands Ministry of Foreign Affairs was charged with conducting that worldwide evaluation.

Description of Actual Services Provided:
Situation assessment and evaluation of the SWAp development process in the sectors health and primary education in Burkina Faso, both at the level of the bilateral Dutch Cooperation as at the level of the receiving government, of Burkina Faso.
The objective of the overall evaluation, as formulated by the IOB, is to assess “whether and to what extent the introduction of the sector-wide approach has improved conditions for achievement of the main objective of Dutch development policy, namely poverty reduction”.
To this end, the following key questions have been formulated:
  • To what extent have the desired changes in Dutch policy been achieved and what explanatory factors can be given for the findings?
  • To what extent have the desired changes in the aid recipient country been achieved and what were the most influential factors?

The evaluation of the Project SANTE/2004/078-547 Programme to Develop Integrated HIV Care for Tuberculosis Patients living with HIV/AIDS has 2 objectives:
  • To provide an independent analysis of the results achieved and the activities implemented in the project co-financed by the EC in Benin and DRC;
  • To make operational recommendations to the Union, the partner countries authorities and stakeholders at NTP and National AIDS Control Programme (NACP) levels as well as the EC, on future measures and strategic lessons with regard to IHC both as operational research and care approach for TB patients living with HIV in developing countries.

  • Briefing at the Secretariat of the Union in Paris;
  • Desk review of policies and IHC documents, interviews and site visit;
  • Workshop in Paris;
  • Final report focussing on relevance, SWOT analysis, effectiveness, efficiency, impact, sustainability, continuum of care, lessons learnt. The focus is on strategic issues for future implementation; One of the main conclusions of the report is that the cooperation created by the project between the national tuberculosis and national AIDS programmes is considered a successful model, which should be appropriated by the different actors in order to maintain the project achievements and to get away from the practice of disease-specific international health financing. The social and economic research component though seems not to have received sufficient attention during the project and hence should be further developed in a second phase.

The project INT108 strives to ‘improve the quality of life of the people living with HIV by strengthening the overall treatment of HIV/AIDS patients within the framework of the decentralisation of the health system.

The evaluation has been realised in a well-documented and very participative way. The local discussions involved all partners engaged in the action.
The mission assessed the efficiency of the health activities, the prospect of viability of the developed health services and of their organisation as well as the adequacy of the project in relation to national and local resources.
The criteria related to overarching themes (combating poverty, good governance, gender, environment) have also been taken into account.

The mission’s main objective was to undertake a final evaluation of MON/002 project and the formulation of the second phase MON/903 for Lux Development. This concerned the following components:
  • Equipment provided, mainly ultrasound and EKG capabilities, necessary to a modern approach of cardiovascular diseases.
  • A medical network allowing exchange of medical information and images between distant provinces and the reference centre in Shastin hospital
  • A specialised web site that provides practical assistance in case management and treatment of cardiovascular diseases.
All institutional and operational aspects had to be reviewed and analysed with the perspectives of a second phase (project coordination, technical assistance, etc).

The primary aim of the Mid-Term Review (MTR) was to assess progress in the implementation of the current strategic plan, identify strengths, weaknesses, opportunities and threats that should be cultivated towards accelerated implementation in the remaining half of the term. Secondly, the review will generate key recommendations that can be used to determine possible modifications of the NHSP on sector-specific interventions in order to accelerate the implementation of the plan to attain the health MDGs.

Description of actual services provided by your staff within the assignment:

The consultant participated in the subgroup Support Systems (SS) and more specifically in the area of Procurement Management System and national drug policy. The team assessed the capacity of the MoH to carry out different phases of the procurement cycle focusing on the organizational aspects; skills of the staff; quality and adequacy of supporting and control systems; record keeping; the general procurement environment and the private sector assessment. In addition the various pharmaceutical policy documents were looked at and assessed. For each area, key questions to initiate discussions during the various interviews were prepared based on the analysis of the available documentation. The questions related closely to the objectives and indicators for the various components of NHSP IV.

Assist the Supply Chain Management system (SCMS) in evaluating the Ethiopia Pharmaceuticals and Medical Supplies Import and Wholesale Share Company’s (“PHARMID”) current procurement capacity; to support PHARMID with the development of procurement tools as basis for the implementation of procurement practices acceptable both nationally and internationally; and to formulate the longer term options for technical assistance to actually achieve implementation.

Description of Actual Services Provided
  • Conduct an in-depth evaluation of the current procurement capacity of PHARMID in general, and for Global Fund and/or World Bank-financed procurements in particular. This review should include the following areas: legal aspects, procurement cycle management, organization and functions, support and control systems, record keeping, staffing, the general procurement environment, administrative, political, and financial risks to the procurement process and private sector assessment.
  • Develop the outline for the standards operating procedures (“SOPs”) for procurement to be further developed and implemented with follow-up technical assistance. This will include procurement procedures for rolling procurement planning, pre-qualification of suppliers and tenders for “framework contracting”.
  • Design the PHARMID organizational structure for procurement with a clear distinction between executive procurement (Procurement Unit) and guiding and supervisory functions (Tender Committee), aligned to the current procurement legislation and regulations.
  • Prepare a detailed work plan for technical assistance in procurement in PHARMID and beyond for the Phase 1: Prepare and Build (Years 1 – 2) and Phase 2: Transition (Year 2).

This project is implemented in nine provinces (Central, East Sepik, West Sepik, Western, Simbu, Enga, Manus, Madang, and Bougainville). NZAID is the sole funder of this project at a total cost of US$2,000,000 over three years (Approx NZ$2.8 million). Funding began in January 2006 and was scheduled to end in January 2009. According to an agreement between NZAID, NDOH and UNFPA, funds for the implementation of interventions under this project have been channelled through the HSIP Trust Account.

Intended project outcomes were:
  • A comprehensive, effective and time-tested comprehensive reproductive health policy
  • Efficient and quality reproductive health services such as safe-motherhood including essential obstetric care, emergency obstetric care, post-natal care and family planning services for women, men and adolescents in the 7 provinces

This review was carried out near the end of a three year funding period, to ascertain the progress the project had made towards achieving its intended outcomes and outputs, to learn lessons and to provide recommendations for the future of the project and the funding relationship. The review covered the period from January 2006 until the present time.

Specific Tasks:
  • Assessment of the national reproductive health situation and determine the appropriateness and relevance of the design including the outcomes, outputs and strategies of the project.
  • Assessment of the extent to which the project was fully integrated into the Health Sector Improvement Programme (HSIP) and the Medium Term Expenditure Framework of the Department of Health including how the project was managed, monitored, reported on and funds acquitted.
  • Assessment of the extent to which the project strengthened the capacity of the Family Health Services Division of the National Department of Health and relevant Provincial and district health authorities to deliver quality reproductive health services.

  • Prepared a Consolidated Field Visit Report based on visits to hospitals and other health facilities to focus on services and strategies in order to improve maternal health;

The Health, Nutrition and Population Sector Program (HNPSP) focuses on (i) accelerating the achievement of health related MDG and PRSP goals through service delivery; (ii) meeting emerging HNP sector challenges (being long-term policy strategies and initiatives in public health); and (iii) implement key reform areas (like public sector management, health sector diversification, decentralisation and stimulating demand for health services).
Various mechanisms have been put in place to monitor its progress, such as the Mid Term review (MTR) conducted in February 2008 and the Annual Program Review (APR) by an Independent Review Team (IRT).

The overall objectives of the 2009 Annual Program Review were:
  • To take stock of the achievements of the implementation of MTR recommendations and progress made on the action plan set out at the MTR;
  • To review the implementation of the HNPSP program vis a vis the revised results framework thus far. Take stock of quantitative and qualitative achievements of the goals, targets, reforms, fund utilization. Recommend revisions to these for the remaining period of the program.
  • To review the financing arrangements and assess how well donor support meets the priorities and requirements of the HNP sector and suggest options for the next HNP support program.
Description of actual services provided by your staff within the assignment:

As a member of the APR Team for HNPSP, the Procurement Specialist provided specific expertise in the management of the supply chain of goods, medicines and services in Bangladesh. He performed a critical analysis of the causes of the observed delays in delivering these products and suggested practical ways to overcome these constraints.
His specific objectives were to
  1. highlight health system issues and challenges in delivering on HNP outcomes, and review the capacity building measures initiated (together with the Human Resources for Health specialist);
  2. make recommendations for strategies, implementation, and financing for the remaining part of the program, to arrive at a road map and a menu of options for the preparation and agreement of support beyond HNPSP.
Followed approach: Briefings with all stakeholders, individual and group interviews, field visits to 4 Divisions/Districts together with representatives of the six Task Groups (TG) from the Ministry of Health and Family Welfare (MOHFW) and the Development Partners (DPs); Obtained information drafted into an ‘issues paper’ that was discussed with senior management of the MOHFW and the DPs; A first draft of the Technical Report and of the Main Report were discussed during debriefing meetings with the TG, the various levels of the MOHFW and the DPs; Support them in formulating new, relevant and practical Action Plans for the next year; Writing of final Technical and Main Report.
