    • The actual state of the health sector and its principal problems;

    • Evaluation of the total health expenditure for the last years;
    • Organisation of the Public health system and of the health care services, SWOT analysis of the public health system, the consequences of the worsening of the political and socio-economical context;

    • Description of the public and private health sector, mentioning for each principal hospital the available services, the demand, the major constraints, the available support, the priority needs and needed interventions;
    • Description of the role and involvement of the Palestinian and foreign NGO’s in health projects;

Assignment: Support to the EU HIV action Plan for Benghazi (Phase 4) in Libya

In December 2007, BRC was awarded a service contract for the provision of management, training and technical support services to the 4th phase of the Benghazi HIV/AIDS Action Plan. The agreement between the EC and BRC will be for the period till 2009, with possibility of extension.

In accordance with EC requirements a “Logical Framework” and indicative workplan were compiled in November 2007. This work plan was subsequently discussed and further operationalised during an inception workshop with Libyan stakeholders and the team of European experts held in Benghazi from 24 to 26 January 2008.

BRC requested HERA to facilitate above mentioned planning process.
The overall objective of the HIV/AIDS Action Plan for the forthcoming period will be to “improve the technical and managerial capacities of relevant Libyan health institutions to deliver an integrated package of preventive and curative HIV/AIDS services”.

Description of actual services provided:
  • Analysis of the proposed logframe. Evaluation of the progress.
  • Institutional analysis and recommendations.
  • Back-to-office report summarizing the inception workshop and making some recommendations to BRC to further improve its performance in Libya.

Preliminary study including an institutional and organizational analysis as well as an analysis of the current funding policies of the health sector.

Description of actual services provided by your staff within the assignment:

For the formulation of the project, one mission in two phases was organised in Burundi.
  1. The organizational and institutional analysis, including among others:
    • A description of the organization of the health system at national level and of Kirundo in particular. Particular attention was given to the institutional capacity at provincial level and the relationship between hierarchical levels;
    • A description of the health system financing at various levels of the administrative services and health care;
    • A description and analysis of all instances involved in the health sector (in Kirundo and nationally), reflecting their role in the sector and the relations between these actors;
    • A quantitative analysis of human resources working in the health sector (public institutions) and their level of training;
    • An analysis of specialized training centres for basic training of health personnel;
    • An analysis of opportunities and initiatives undertaken by stakeholders to promote the harmonization of interventions in the health sector in order to improve efficiency (including a description of the policy and modes of intervention for other donors in the sector);
  2. An analysis of the current funding policies of the health sector included:
    • An analysis of the modalities and operational problems of the financing system for the decentralized health structures, and the coordination with the central level;
    • The identification procedures for contracting;
    • Identifying areas of uncertainty and risk in the new funding policy.
The team carried out the above described tasks meticulously and presented their results at a workshop for all local and national stakeholders involved in the health sector and other interest fields. This workshop helped to validate the key proposals.

The project focuses on strengthening the health system in terms of geographical accessibility and improving the delivery of health services in the wilaya of Tamanrasset across 3 areas:

  • Decreasing the levels of mortality and maternal and neonatal morbidity
  • Strengthening the management of reproductive health, particularly in the field of family planning and adolescent health and youth.
The formulation mission aims to validate the data contained in the identification document and to prepare a Technical and Financial Proposal (TFP)

Description of actual services provided by your staff within the assignment:

The formulation team has worked closely with experts from the Ministry of Health, Population and Hospital Reform and with the representation of BTC in Algiers on the following activities:
  • Validating the data of the identification mission, namely the validation of the project objectives and defining (or adjusting if necessary) the results. For this purpose, the objectively verifiable indicators, along with the sources of verification and the main assumptions which could jeopardize the implementation of the project, were specified.
  • Presentation of the logical framework, including a detailed description of activities with indicators, sources of validation and critical assumptions;
  • Produce a Technical and Financial Proposal (TFP) on the basis of the logical framework with
    • Detailed planning
      • validating the activities related to the results
      • planning of the project based on expected results and activities
      • Defining the timeline
      • budgetary planning in accordance with the provisions of BTC
    • Allocation of financial and human resources to the governments of Algeria and Belgium;

Assignment: Formulation of the project to support the multisectoral national plan to combat HIV/AIDS

In 2001 Burkina Faso and the Belgian government initiated the identification process of a multisectoral HIV/AIDS intervention. The identification report was finalised and validated in 2003. After that, a team of 3 consultants was contracted by BTC for the formulation of the project.

The general objective of the identification report was “the negative impact of HIV/AIDS on sustainable development of Burkina Faso is reduced”, while the specific objective was “the implementation of the Multisectoral National Plan to combat HIV/AIDS has improved in a sustainable way”.

The proposed project consists of three “subprojects”, each of them with a specific objective:
  1. a subproject that has the objective to reinforce the multisectoral fight against HIV/AIDS in the region of Plateau central;
  2. a subproject that has the objective to reinforce the response to HIV/AIDS in university community of Ouagadougou; and
  3. a subproject that aims at improving the operational research in the field of HIV/AIDS in Burkina Faso.
Description of Actual Services Provided:
  • Analysis and validation of the identification report.
  • Analysis of the problems and identification of solutions in a participative workshop.
  • Study on the capacity of SP/CNLS to coordinate the whole project, the capacity of putting the right human resources at the disposal of the project (integration of Centre Muraz) and the necessary financial means.
  • An analytical review of the proposed strategies in order to select the most accurate to obtain the global goal of the programme and to diminish the number of interventions.

The team formulated proposals for adequate support to the Primary Health Care (PHC) in Karagwe within the frameworks of both the KCDP and the Health Sector Reform (HSR). The proposals were based upon an analysis of:
  • The need for adequate financial contributions from the government, interested donors and communities;
  • The current, basic medical service delivery systems; identify how the PHC in general can be supported;
  • The current operational and financial management systems in the health facilities;
  • In particular: addressing the prevailing situation of regular shortcomings in the drug supply system.

Due to the increase of budget allocation and new time frame for the above mentioned project, the existing formulation document must be adapted and new conditions must be budgeted.

Description of Actual Services Provided:
The expert considered all the already existing documents in order to produce some necessary changes. He adapted the proposed activities to the new budget and time frame based on the recommendations from the advisory committee and on inputs from the Representation in Tanzania. Furthermore he paid special attention to the long term approach for cost sharing and health insurance.

The first phase of the study focused on (i) the setting of the cooperation intervention in partnership with the province as it is conceived in the Republic of Vietnam nowadays, (ii) analysis of the health sector in the province of Hoa Binh, (iii) validation of the identification report in the actual context and (iv) the consensual definition of goals, strategies and expected results of the project.
The second phase focused on the elaboration and presentation of the preliminary technical and financial file.

The team did the evaluation of the support programme of Memisa, including questions regarding the objectives, indicators, budgets, needs of the population, the strategies, the coordination, etc. Recommendations were included in the report and formed the basis of the formulation of the fulltime 3 year programme. The team held a restitution meeting with the partners and the representatives in order to share the conclusions and recommendations.

The works built on the identification work undertaken so far by the EC, the recommendations from the Public Finance Mission, project preparatory studies undertaken by other donors and complementary investigation as required. Focus was on rationalising the allocation and the use of resources for health in adopting a sector-wide approach, explicitly acknowledging the EC support programme.

Description of Actual Services Provided:
Design of the EC support in such a way that there is no overlap with activities financed by other donors (in particular ADB, World Bank, GTZ, KfW and JICA) and with maximum of complementarity.

The 3-year EC support entails 3 components:

The European commission has been active in the health sector in Mozambique for many years. The modality of EC support has gradually changed from project support towards sector wide support. EC support aims at strengthening health systems through systems reform and related capacity building as well as through direct financial support. In order to identify appropriate EC support under the EDF-10 to strengthen institutional capacity in the health sector, a needs assessment for capacity building and a mapping of these needs was undertaken.
The objective of this consultancy was to provide advice on the outline of the next EC health and HIV/AIDS Sector Policy Support Programme in Mozambique (2009-2011), specifically in relation to the needs for capacity development for the National AIDS Council and for the Ministry of Health. Options for financial support to CNCS and sector budget support under the PROSAUDE II financing modality were explored.

Description of actual services provided by your staff within the assignment:

The following activities were performed in order to achieve the mission objectives:
  • The team met with key officials from MOH, CNCS, to identify perceived needs for support, to make a situation analysis of ‘systems development’ within MOH and CNCS, and to map available technical assistance and needs for institutional capacity building.
  • A desk study of relevant documentation on the development of the health sector, including the development of SWAP was undertaken;
  • The team assessed the extent to which EC eligibility criteria for further support were met.
  • It was suggested to include five areas of support in the EC support plan (SPSP):
    1. Proactive participation of the delegation in the MoH led sector wide approach;
    2. Financial support through the sectoral ‘Proasaude’ on-budget common management arrangement;
    3. Development of appropriate, innovative and effective approaches of institutional capacity building, including twinning arrangements with relevant public health institutions;
    4. Technical assistance to support institutional development at all levels in the health sector.
    5. Technical support to the CNCS to support an effective national response to the HIV/AIDS epidemic.
  • The team proposed to provide this support through three modalities:
    1. Financial support to Prosaude, according to national management procedures;
    2. Delegated support to CNCS, by involving a EU Member State;
    3. Specific EC procedures for providing TA.

The objective is to assist the Province of Luxembourg in deciding on an appropriate course of action, the objective of this particular assignment is to make the ‘case for change’. Within the scope of meeting the economic and financial objectives, the ‘case’ shall build on current international trends and experience in hospital service rationalisation and take the Province de Luxembourg’s particular circumstances into consideration. The ‘case’ may include one or two options to the Province’s current hospital configuration - broadly elaborated and based on relevant, applicable practice. To further assist in decision-making, the assignment will include a ‘road map’ setting out the steps, the work, the timescale, and the likely cost of identifying and fully appraising a preferred hospital configuration for the Province.

Description of Actual Services Provided:
  • a basic survey of the health institutions in PdL – to assemble the essential knowledge of existing institutions’ capacity, functionality, services offered, staffing, catchment area, design, state of repairs, management, and financial situation etc;
  • a broad assessment of the demand for hospital services in the Province, based on epidemiological data or deriving from a similarly accepted methodology;
  • an estimate of required hospital capacity in the Province. The estimate will take due note of current international trends in hospital services provision and - to the extent possible – take into consideration PdL inhabitants’ access to services outside the Province and the need to continue to procure certain specialised services elsewhere;
  • an analysis of the ‘problem’ and a reasoned suggestion on a response. The pros and cons of the option(s) as concerns investment cost, recurrent cost, accessibility, quality of services, staff requirements, etc. should be described in approximate terms comparative to the current situation;
  • a ‘roadmap’ of the of how to move towards identifying and selecting a preferred option. This will include a detailed description of the tasks included, their time scale, and their approximate cost (=terms of reference), and advise on other practical steps in the process.

Assignment: Identification and Formulation of a project for the renovation of the central hospital in Beira

The Ministry of Health (MISAU) had applied for Flemish Cooperation support funds for the rehabilitation of an Infectious and Contagious Disease Ward and for improving laboratory examinations in Beira. The identification and formulation mission was supposed to check the relevance, feasibility and sustainability of the proposal for the renovation of the central Hospital and the “21 de Julho” Hospital.

  • SWOT-analysis
    The strengths/weaknesses and opportunities/threats were analysed for both alternative solutions: (1) to rehabilitate the building of the hospital ‘24 de julho’ and transform it in a reference unit for infectious diseases; (2) to build a new ward inside the hospital compound that would serve both as the TB and dermatology ward and an infectious disease reference clinic The conclusion was that the construction of a new ward was the best alternative.

        The global objective was to analyse the statistics and service delivery monitoring capacities within the MoH, specifically through the implementation of the HMIS, and design a project proposal for capacity development to assist MOH to run a service delivery monitoring system. The more specific objective was to assess the existing institutional strengths and weaknesses of the current HMIS as well as to identify the priority needs for capacity building

        Description of actual services provided:
        The consultant assessed the existing institutional strengths and weaknesses as well as identified the priority needs for capacity building. Therefore the expert:
        • Studied the HMIS format, templates and flows as approved by MOH in March 2004;
        • Obtained and reviewed all relevant project documentation of donors currently involved in the implementation of the HMIS, including ADB, WB, Lux Development and WHO and held discussions with them to ensure complimentarily of EC support;
        • Reviewed WHO updated HMIS implementation strategy with different stakeholders;
        • Close collaboration and support of the Division of Statistics, Department of Planning and Finance. Identify their needs and propose areas where technical assistance of the EC could be constructive;
        • Identify training needs of health staff to use HMIS data collection forms at Central, Provincial, District and Health Centre levels; Identify needs for improved utilisation of HMIS for evidence-based management at every level of the health pyramid both at facility level as well as health office level;
        • Undertake two field visits;
        • Assess the level of computerisation in the use of HMIS at different levels and the plans in the near future to increase computerisation and upgrade software;
        • Liaise with the unit in the Prime Minister Office managing the national e-governance programme in order to identify the data that the HMIS can provide;
        • Liaise with the National Statistics Centre to identify any needs in terms of increasing the information sharing between MOH and NSC.
